Application Procedures

How do I apply for a US passport?

There are four types of passport application forms.  The form you use will depend on the type of application being made.

1.  DS-82:  Form DS-82 is used for the renewal of adult passports and for applicants requesting, within one year of a passport’s issuance, a name change, an extension of validity for limited passports, or a correction of a printing error.
2.  DS-5504:  Form DS-5504 is used by applicants requesting a replacement passport within one year of the issuance of a limited passport.
3.  DS-11: Form DS-11 should be used for all first passport, minors (ages 16 and under) new and renewal and Lost/Stolen passport applicant.
4.  DS-4085: Form DS-4085 should be used by applicants requesting additional visa pages.

In order to use the DS-82 application form, you must meet all of the following criteria:

• You must have been issued a U.S. passport in your name within the past 12 years
• You must submit your most recent U.S. passport with your application
• You must have been age 16 or older when your most recent U.S. passport was issued
• A previous U.S. passport is adequate evidence of both identity and citizenship.  In lieu of a previous passport, identity can be proven with a photo I.D. issued by a competent authority.  U.S. citizenship, however, must be established by presenting either a certified copy of a U.S. birth certificate, a Consular Report of Birth Abroad (if the applicant was born outside the U.S.), or a U.S. voter registration card.  Federal law prohibits the issuance of a full-validity U.S. passport to any person who lacks documentary evidence of U.S. citizenship.

In addition to the completed passport application form, you will be required to submit:

• Two photographs measuring 2" x 2" (5cm x 5cm)
• Proof of your identity
• Proof of your US citizenship
• A passport application fee (see "Fees" below)
• Application that does not meet all of these criteria should be made using the DS-11 form, which requires the applicant to make a sworn statement before a consular officer

How do I apply for a passport for my child?

• A completed form DS-11 (Do not sign until the officer directs you to do so.)
• Proof of U.S. Citizenship of the child which could be U.S. Passport, a certified copy of a U.S. birth certificate, or a Consular Report of Birth Abroad (if the applicant was born outside the U.S.).
• Parental proof of relationship to the child (For example, a certified foreign birth certificate, adoption decree, court order establishing custody, etc.).
• Parental Identification (passport, driver’s license, etc.).
• Both parents must appear together and sign the application or when only one parent appears, the parent should sign and submit a primary evidence of sole authority to apply, such as child’s certified U.S. or foreign birth certificate listing only the applying parent or a death certificate of non-applying parent.  If none of the above documentation is available, the applying parent/guardian should submit Form DS-3053: a Notarized Statement of Consent: Issuance of a Passport to a Minor Under Age 16.
• Each minor (child under age 16) applying for the first time or for a renewal passport shall appear in person.

Passport Fees

Passport applications for children under age 16 cost U.S.$ 85.00.

A first-time application (DS-11) for an adult (age 16 and above) costs U.S.$ 100.

An adult renewal application (DS-82) costs U.S.$ 75.00.

The fee for a replacement of a lost or stolen passport is U.S.$ 100.00 for adults and U.S.$ 85.00 for children under 16.

All fees may be paid in U.S. or Mauritian rupees in cash.  Please note we do NOT accept checks or credit cards.
